Code of Conduct - Incident Reporting Form

To further improve our process for implementing, enforcing our Code of Conduct, we have created an Incident Reporting Form that any member of LMYA may complete to report an incident to the Lower Macungie Board Members relating to inappropriate behavior by any Coordinator, Coach, Parent, umpire or spectators at any LMYA event.

We encourage everyone to follow our Code of Conduct and avoid league involvement in conflict resolution, but we continue to have issues each year and we need to streamline the process.
